An application form can be downloaded from the UNISON website, and you can also phone 0800 0857857 (6am-midnight Monday-Friday and 9am-4pm on Saturdays).
All application forms should be returned to Angela Jelfs (either directly or via the applicant’s local steward/workplace rep) and not sent directly to UNISON.
